How to Run an Exception-Based Operations Review

An exception-based review protects management attention by focusing discussion on variance, risk and decisions instead of repeating every normal activity.

THE FIRE

When every line receives equal airtime, critical issues compete with routine updates. The team leaves with more minutes used but no clearer priority.

THE BLUEPRINT

1. Set the expected condition and threshold for escalation.
2. Review only exceptions that exceed the threshold or threaten the next output.
3. Assign the decision, owner and follow-up evidence before moving on.

THE METRIC

Measure review duration, exceptions resolved, unresolved actions and repeat exceptions. A good review becomes shorter as the system becomes more reliable.

Put this into practice

Have the reporting owner prepare this before management review, with the receiving decision owner available.

Match Expected position and Actual position to one cutoff; label assumptions rather than presenting them as verified causes.

Begin with one example using Expected position, Actual position, Impact, Decision required, Decision deadline. Give the record a stable identifier and connect any unresolved issue to an owner and next action. Ask the receiving reviewer to explain the decision the record supports before expanding the process.
